【Notice to People Enrolled in National Health Insurance】【关于国民保险】【Para inscritos no Seguro Nacional de Saúde】【Thông báo về Bảo hiểm Y tế】【国民健康保険（こくみんけんこうほけん）に　入（はい）っている人（ひと）へ】

You will receive two letters by mail from the City Office during the third to fourth week of July.
One comes in a gray envelope. It states the amount of National Health Insurance tax you must pay for the coming year and includes a payment slip for use at convenience stores.
The other comes in a light blue envelope. Those who use their Individual Number (My Number) card as their health insurance card will receive a "Notice of Eligibility Information" (Shikaku Joho no Oshirase). Those who do not use their My Number card as their health insurance card will receive an orange card called an "Eligibility Confirmation Certificate" (Shikaku Kakunin-sho). Please be sure to open the envelopes and check the contents.

7月的第3～4周，市政府将邮寄两封信给加入国民健康保险的人。其中一封是灰色信封，写有今后一年的国民健康保险税金额，并附有可在便利店缴费的缴款单。

Na terceira ou quarta semana de julho, você receberá duas cartas da prefeitura.
Uma delas estará em um envelope cinza. Nela constará o valor que você pagará de imposto do Seguro Nacional de Saúde. Também conterá boletos para pagamento em loja de conveniência.
A outra carta estará em um envelope azul claro. Se você utiliza seu My Number como cartão do seguro saúde, receberá uma [Notificação de Informações]. Quem não utiliza o My Number como cartão do seguro saúde, receberá um cartão laranja chamado [Shikaku Ninteisho]. Certifique-se de abrir o envelope e verificar o conteúdo.

Lá thư thứ hai là phong bì màu xanh nhạt. Nếu bạn đang sử dụng thẻ My Number làm thẻ bảo hiểm y tế, bạn sẽ nhận được giấy "Thông báo thông tin tư cách tham gia bảo hiểm". Nếu bạn không sử dụng thẻ My Number làm thẻ bảo hiểm y tế, bạn sẽ nhận được "Giấy xác nhận tư cách tham gia bảo hiểm", là một thẻ màu cam.
Hãy nhớ mở phong bì và kiểm tra kỹ nội dung bên trong.

７月（がつ）の第３～４週（しゅう）に、市役所（しやくしょ）から手紙（てがみ）が２つ　郵便（ゆうびん）で　届（とど）きます。
一つはグレーの封筒（ふうとう）です。これから１年間（ねんかん）国民健康保険税（こくみんけんこうほけんぜい）をいくら払（はら）うか書（か）いてあります。また、コンビニで払（はら）うための紙（かみ）も入（はい）っています。
もう１つは、水色（みずいろ）の封筒（ふうとう）です。マイナンバーを保険証（ほけんしょう）として使（つか）っている人（ひと）には、「資格情報（しかくじょうほう）のお知（し）らせ」が届（とど）きます。マイナンバーを保険証（ほけんしょう）として使（つか）っていない人（ひと）には、「資格確認書（しかくかくにんしょ）」というオレンジ色（いろ）のカードが届（とど）きます。必（かなら）ず封筒（ふうとう）を開（あ）けて、内容（ないよう）を確認（かくにん）してください。
